I'm contacting InfringingWeasels.com in regards to a copyright violation. Below is all the pertenant information you require regarding DMCA protocol along with a few other points of information I need to have answered as well regarding the infringed artwork.

1. A physical or electronic signature of a person authorized to act on behalf of the owner of an exclusive right that is allegedly infringed.

This email acts as my electronic signature. I have also attached a scan of my signature. This should suffice.

2. Identification of the copyrighted work claimed to have been infringed, or, if multiple copyrighted works at a single online site are covered by a single notification, a representative list of such works at that site.

5. A statement that the complaining party has a good faith belief that use of the material in the manner complained of is not authorized by the copyright owner, its agent, or the law.

I'm the creator and copyright owner of the artwork in question. At no time have I granted usage or licensing of this art to a second party to sell on InfringingWeasels.com.

6. A statement that the information in the notification is accurate, and under penalty of perjury, that the complaining party is authorized to act on behalf of the owner of an exclusive right that is allegedly infringed.

All statements and information within this email are true.

7. In addition to the minimal DMCA requirements I also require the following information to be provided to me the copyright holder of the artwork in question.

- Full contact information (Name, Address, Phone, Email) of person responsible for uploading my artwork to InfringingWeasels.com web site.

- Notification of all parties who downloaded this artwork and informed of violation and refunded accordingly

- InfringingWeasels.com will notify Your Business Name Here when this has been done
